What you need to know on implementing the Preventing Chronic Disease Action Plan of the Prevention Agenda 2013-17!

The prevention Agenda 2012-17 is the blueprint for state and local action to improve the health of New Yorkers in five priority areas and to reduce health disparities.

The New York State Department of Health (NYS DOH) is developing a series of topic- specific webinars and in-person regional training opportunities on chronic disease prevention.   The trainings will ensure that local and public health partners have the knowledge, tools and resources needed to implement evidence- based strategies that address chronic disease, align with the prevention agenda 2013-17, and are consistent with NYSDOH priorities. The schedule  for the webinars is listed below:

September 17, 2013, 1:00pm– 2:00pm
Barbara Wallace, M.D., M.S.P.H., Director of the Division of Chronic Disease, will provide an overview of the burden of chronic disease in New York State and the evidence based strategies that are included in the Preventing Chronic Disease Action Plan of the Prevention Agenda 2013-17.

An overview of four evidence based strategies that support the goals of the Prevention Agenda will be discussed and how they will be supported for inclusion in Community Health Improvement Plans and Community Service Plans. These strategies include:

· Increasing adoption and use of food standards by venues reaching priority populations including municipalities, community-based organizations, worksites, and hospitals;
· Implementation of Complete Streets policies, plans, and practices;
· Promotion of policies and practices in support of breastfeeding exclusivity and duration in hospitals, primary care, and worksites; and
· Increasing the availability, accessibility and use of evidence-based interventions through promotion and referral including the Chronic Disease Self-Management Program (CDSMP), the Diabetes Self-Management Program (DSMP), Diabetes Self-Management Education (DSME), Diabetes Prevention Program (DPP), the Arthritis Foundation Walk With Ease Program (WWE), Active Living Every Day (ALED) and Asthma Self-management Training (ASMT).
